A Clinical Study of the Diagnostic Accuracy for Acute Appendicitis


Abstract
This report is a retrospective study on 320 patients operated under the impression of acute appendicitis at the department of surgery, Min Joong Hospital, Kun Kuk University during 1 year period form May 1990 to April 1991.
The purpose of this study is to identify and evaluate the diagnostic accuracy for acute appendicitis in the association states with sex, age, right lower quadrant abdominal tenderness. rebound tenderness. fever and leukocytosis.
@ES The results were as follows ;
@EN 1) The diagnostic accuracy for acute appendicitis was 80.3% in all patients. The sex differences of the diagnostic accuracy in each age groups were not significant, but the sex difference in total patients was significant(p<0.05), such as
85.0%, in
male and 74.3% in female.
2) The diagnostic accuracies according to the age groups were 88.1% in 1~14, 76.2% in 15~29, 78.5% in 30~14, 92.0% in 45~59 and 85.7% in over 60 year old group. But no difference was noted between the diagnostic accuracy in any age group and
that
in
the other patients.
3) A significant difference was noted between the diagnostic accuracy in patients with rebound tenderness, 87.8% and that in patients without rebound tenderness, 68.5%(p<0.01). The difference of the diagnostic accuracies between 87.3% in
patients
with
fever and 73.6% in those without fever was significant(p<0.01). And the difference between 92.0% in patients with leukocytosis and 57.0% in those without leukocytosis was also significant(p<0.01).
4) According to the analysis of the diagnostic accuracies at the combined states with or without rebound tenderness. fever and leukocytosis, the accuracy in patients with all findings was 95.5% and that in patients without all findings was
37.5%.
The
former was the highest group and was significantly higher than that in the other patients(p<0.01). The latter was the lowest one and significantly lower than that in the others(p<0.01).
